My accountant made an extensively large error on my tax return resulting in me having to pay back a large sum of money to the ATO 11 months after lodging my return. The error was a simple typo by the accountant however, although they accepted responsibility, they did not offer a refund on the $95 fee to do the return. All they offered was a discount to do my next return remotely (not from my home as per standard arrangement). They always come to my home to do the return so this offer was insulting.
