If you elect to pay the annual membership monthly, download the application form here and return it to ICB. Certificate of Currency of your Professional Indemnity Insurance (No PI?, Apply for PI here). Copy of your Qualification Certificate / Statement of Attainment / transcripts (if applicable). Membership Declaration & Certification statement must be completed either online or download here, complete and return.
