These can be filled in on the day of the appointment, however you will need to allow time for this. PDF format of the consent form, or to download the. Word format of the consent form. It is important that you complete these on the day of your vaccination, as some questions are time specific.

Patients' booking with a Doctor they haven't previously seen or who isn't taking new patients may be rebooked with another Doctor. Poor communication, lack of care, absolutely abysmal service from the doctors and practice manager and super unknowledgeable. After a horrible experience, booked a meeting with the practice manager, and she refused to show up, and didn't organise it with the doctor. I could not recommend this practice to anyone.

My family and I have been patients of North Nowra Medical Practice for about 10 years. My kids and I have always been treated so well by our Primary Dr, Dr Anju Hanchate. She has built a great relationship with us all, especially my teens who trust her 100%. I highly recommend Dr Anju for teens with mental health. She never makes us feel rushed or like our issues don't matter.
North Nowra Medical Practice are also very fortunate to have an awesome Practice Manager. Nic is often extremely busy but I have never had a problem she has not been able to solve in a very timely and efficient manner. If

Waiting times are excessive, many receptionists are overworked and surly, and those GPs still accepting new patients aren't as competent or compassionate as they should be. One GP convinced a relative that removing an ingrown toenail was a simple procedure then proceeded to forget crucial steps, botching the surgery, and creating longterm pain and a mangled toenail.

I have tried twice to get travel vaccinations. The first attempt I rang the day before my appointment to confirm date and time, was told there was no record of an appointment and told to reschedule I did for the next day.
I repeated the time and date with the receptionist entered it in my phone.
We arrived at the practice was told there was no appointment AGAIN.
The receptionist went to check with the nurse then the doctor came back and told me the doctor said to reschedule. Not bloody likely, if they can screw up two appointments why would I trust their ability to not screw up the vaccinations
