Previously known as Enhanced Primary Care Plan. Please liaise with your GP who can prepare your child's Chronic Disease Management Plan. This provides a partial Medicare rebate for up to 5 sessions of allied health services over 12 months. The NDIS provides support to eligible people with intellectual, physical, sensory, cognitive and psychosocial disability.
